Add PersonalAccessToken::RevokeService

What does this MR do?

• Adds a PersonalAccessToken::RevokeService that revokes personal access tokens.
• Refactors the route for revoking tokens in the web UI to use this new service.

This is an iterative step towards completing this issue.
