Update upgrade policy to reflect new major version

Balasankar 'Balu' C requested to merge add-13-to-upgrade-recommendation-docs into master

As per our support policy, we support current major version and two previous ones. Let's make the upgrade recommendations reflect this, and add an example for someone to jump to the new major version.
