Allow Duo code review to approve or request changes
What does this MR do and why?
Duo code review approves a merge request when a review finds nothing,
and requests changes whenever it leaves any comments. A standing
request stays in place until a clean review clears it, and starting a
new review removes Duo's earlier approval so the new review can be
recorded. Its approval satisfies the Any approver rule only when the
nested counts setting is on.
