Fix hot-reload on Rails 7.2 with full path to ConnectionProxy

What does this MR do and why?

Rails hot-reload was causing crash loops as of the upgrade to Rails 7.2 (Upgrade Rails to 7.2.3 (!212905 - merged)); @heinrich @engwan (I am so sorry) and I managed to track down the cause to our connection load balancing proxy monkey patches.

The fix is using a full path to ConnectionProxy, since it's now being run in a context where that name doesn't get tried by default.
