Refactor sign in HAML form
What does this MR do and why?
Preparation for &19615
In #574982 we will be migrating the sign in form to Vue. To keep things simple (and because it doesn't need to be dynamic) we will leave the Enter admin mode form in HAML. To make this migration easier this MR refacotrs the HAML template so it is split up in a way that is easier to migrate just the sign in form.
