What does this MR do and why?

https://docs.gitlab.com/administration/gitlab_duo_self_hosted/ contains nice tables of features that show which features are supported with Duo Self-Hosted. However, this Duo documentation page contains info that would be misleading or confusing since it doesn't make a distinction about whether GitLab or the user team is hosting Duo.

This attempts to clarify which Duo features are available for GitLab Self-Managed customers that also use Duo Self-Hosted.
