Store description versions

What does this MR do?

Store versions every time the description of an issue / MR / epic changes.

Also associates these to the generated system note so that a diff can be shown in the future.

• The MR includes necessary changes to maintain consistency between UI, API, email, or other methods
• Security reports checked/validated by a reviewer from the AppSec team
