Add column to approval_group_rules table

What does this MR do and why?

Add applies_to_all_protected_branches column to approval_group_rules.
This is a boolean to store if a approval group rule applies to all protected branches as specified in &11451.

Contributes to Extend ApprovalRules::CreateService to support ... (#430096 - closed).
