Remove avatars from user profile activity
What does this MR do and why?
- Remove repeated usernames, names, and avatars on user overview and activity tabs
- Update styling
- Add vertical line to match MR/issue activity
- Add missing approval icon
- Keep icons visible on mobile
- Change icon color to gray
