Fix service discovery refresh-hosts delay

What does this MR do and why?

Currently for refreshing LB hosts, it might sleep for a maximum of 2 mins (conditional loop) at the worst case (which seems to occur most of the time).

This especially creates elongated waiting time (~40 mins) for PG upgrades, because all the old-hosts have to be rotated with the new ones.

Instead of waiting for 2 minutes per host, try to gracefully disconnect all hosts in parallel for 2 minutes, and then force-disconnect each one that is not successfully disconnected.
