Adapt MR widget to support fail-closed approval rules

What does this MR do and why?

This MR is backporting a revert-MR !118140 (merged). The revert came 1 day too late and it didn't make it into the 15.11 and as a result of that, some changes to the MR widget were released from !117423 (merged) which are causing cosmetic inconsistency. The customers are noticing this: !117557 (comment 1383654845).

These inconsistencies were reverted to give enough time for backend fixes and the feature itself (#389905 (closed)) will be released in 16.0.
