Handle duplicate/stacked achievements better

At the moment a user profile could potentially just show 3 of the same achievement. The hover functionality will only work for the first.

We should at minimum fix the hover... but maybe further improve by "stacking" them to just show once with tooltip like: Awarded x times by namespace
