Rollout of `Gitlab::Database::Migration[2.0]` with `RestrictGitlabSchema`

This issue describes all outstanding bugs and tracks fixes to implementation of restrict_gitlab_migration gitlab_schema:.

This issue is to add documentation for the usage of migrations in context of multiple databases.
