Improve alt tags on user uploaded images

With all user uploaded images, we set the alt tag in markdown, but most of them (at least for me) are the "Screen shot" text. I think we could improve these alt tags to improve our accessibility. There are some auto-alt-text tools we could utilize to run during upload.
Background
After going over this part of the course, I created an issue around how we can improve our alt tags of images posted in markdown comments: #321436. Currently, I just take screenshots and post to comments, however, the alt tags just read Screen_shot.... I thought about how we could potentially get "smarter" on how we update these alt tags if the alt tag doesn't describe the image. I think this is something we should look into because I have a feeling a lot of use just post images that are screenshots with no thought into what the alt tag is.
There are ways to add alt text to markdown images (although we could certainly make improvements to better prompt/guide users to doing so), but what about the design management feature? AFAIK, there is no way to add alt text to designs in design management.
I could see this for alt text too. Twitter at least provides a UI for adding alt text to images that you upload, I wonder if we could do something similar.
