Clean up error handling for approval rule creation
Problem
Approval rule modal validation errors don't show up in the modal. They show up as a banner on the parent page. Since the modal has a darkened background and the banner is at the top of the page, it's not obvious there is a validation error.
It seems like the form is broken when a validation error occurs.
