Impact assessment - summit challenge free health check

Overview

At Contribute 2018 a "connect with sales" option was added to self hosted instances that were free and had over 100 users. The actual copy of the "connect with sales" link was "Get a free instance review" and a screenshot of the experience is listed below. The goal of this issue to asses the impact of this change.

Questions to answer

• Confirm this is still live in production
• Confirm the SFDC report listed above is accurately counting the number of leads captured via this cta. Currently the report only lists 28 records.
• Determine how much if any IACV was generated from these leads after they clicked the CTA
