refactor: rename CodeSuggestionsStatusBarItem to DuoStatusBarItem

Description

Rename CodeSuggestionsStatusBarItem to DuoStatusBarItem to better reflect that the status bar item represents the overall Duo feature state, not just Code Suggestions.

Changes:

  • Rename class CodeSuggestionsStatusBarItem → DuoStatusBarItem
  • Move file from src/common/code_suggestions/code_suggestions_status_bar_item.ts to src/common/duo_status_bar_item.ts
  • Rename test file accordingly
  • Update all imports and references across the codebase

This is a pure refactor — no logic changes.

How has this been tested?

  • If src/browser or src/common has been modified, please consider interoperability with the Web IDE. See Running the Extension in WebIDE.
  • Consider an end-to-end test for significant new features that aren't covered by integration tests.

All existing tests have been updated to use the new class name and file path. The rename is verified by checking all references in the codebase.
