Ensures all anchor elements with an href attribute inside .md
(markdown-rendered) content display with an underline, consistent
with accessibility best practices and the existing external-link
underline behaviour.

The underline rule for links inside .md blocks belongs in the markdown
stylesheet rather than the typography stylesheet, since it is specific
to markdown-rendered content.

Before, the Lookbook embed script was loaded on every page, even though
most pages don't have Lookbook embeds. Now, the script is loaded on
demand at most once during a session. If for some reason the script
fails to load, an error is displayed with a link to the external
Lookbook page.

The larger benefit of this is that the whole site doesn't break if the
Lookbook app is unavailable.
