Fix markdown links in PROCESS.md

What does this MR do?

This MR fixes the markdown links in PROCESS.md, which were rendered as plain text and not links. For some reason the brackets [, ], ( got escaped.

It also fixes a link to definition-of-done section in CONTRIBUTING.md and escapes the ```.
