Update process to create new design artifact issue

As discussed in this week's product meeting, we should make sure to create new issues for ~"design artifact" to prevent misunderstanding that the actual feature has been scheduled for delivery. We should follow this process for all issues, not just direction or Product Vision 2018 etc.
