Remove Channel and Username fields for Slack Service

Since Slack allows you to set this information in their Service Configuration panel I suggest we drop support for it in GitLab. That way it is more clear where you need to change information. Since at this point GitLab will override information you setup in Slack. Which causes weird situations.

Lets have Slack deal with Service configuration, we just send our payloads to them.
