Present GitLab version for each V3 to V4 API change on v3_to_v4.md

To give a better trackability of what exactly is in production and what's not on https://docs.gitlab.com/ce/api/v3_to_v4.html, would be nice to present the GitLab version which each change was/will be introduced.
