Small CSS Followups

A small selection of CSS fixes for #775 (closed), various pieces and:

Also, the TOC items are a little small and close together and I mistapped an incorrect item a few times. Finally, this long URL string not wrapping is the sole reason there is ~50% extra width for this very long page.
