Include severity labels

Our UX scorecard template part 2 asks to create UX scorecard-rec issues based on the evaluation, but we don't mention anything about prioritizing those issues/epics created. We started doing this lately, as it was related to an OKR, but I believe this will help us in the long run to prioritize and clearly communicate the impact of an issue on users.
