operations: Remove SquashUsingMerge feature flag

Since the SquashUsingMerge feature flag has been running in production without issue, it should be OK to remove it now.

Rollout issue: #4213 (closed)

I've also tweaked a couple comments since we're no longer using rebase for squashing.
