Use scanned commit hash to reference findings

This adds two user-facing changes:

• Uses the scanned commit hash to link to code findings instead of master (i.e., permalinks).
• Updates the subheading of the page to reflect the scanned commit.

There are various refactorings involved as well. Read the commit messages for more detail!
