This series of changes extends the multi-approval configuration
ability for MCP tools. Documentation surrounding tool approval
was also added and a demo was added to test the various config
options live.

This change moves the approvalOptions changes to the
message_tool_approval component since the agentic approval
flow component has been deprecated.
