What does this MR do?

It fixes three bugs in the tool approval UI:

  1. Ensures that the create_commit tool doesn't break when the tool's info doesn't include the commit actions.
  2. Simplifies the message for the create_note tools.
  3. Fixes a text-overflow for the run_command tool.
