Add self_hosted_duo_agent_platform add_on and UP

What does this MR do and why?

This MR adds a Unit Primitive and AddOn for self_hosted_duo_agent_platform. Since a UP and AddOn can't share the same name, the AddOn has add_on attached to the end of its name.
