Add issue template for k8s version upgrade process

As part of the process to support the next Kubernetes versions, we want to have a guide on which of our applications we might need to update.

Having this issue template can serve as such a guide. This is still an initial change, and follow-up MRs could be added to make this clearer based on how the work goes for gitlab-org/gitlab#406711 (closed)
