Update Quality template to note new discussions should be created

Mark Lapierre requested to merge update-quality-template into master

This updates the Quality template to be closer to the default template used by other teams.

It instructs us to start a new discussion for each point, making it a lot easier to have discussions because they're no longer all in the same thread.
