Add option to discuss issues that greatly exceeded estimation

This MR adds a section to the retrospective for re-evaluating issues where the estimation was far off, along an associated emoji. We can then add a 🎤 for ones we want to discuss, and possibly have a follow-on session on those and try to understand what we missed and why we missed them and how we can improve on them.
