Update guidance around making navigation changes

Why is this change being made?

Updates guidance for making left sidebar changes, and links to new issue template. This change is being made to streamline navigation changes and ensure that all changes are being looked at holistically. It also adds a DRI for making changes to the top-level menu items, ensuring that one person can be a point of contact for decision making.
