Incident triage and assignment process

Why is this change being made?

For our FY22-Q2 OKR [Ally] we're looking to establish a method for triaging incidents and assigning those incidents to teams. As an initial MVC, the idea was to take the current process that is described in the incident review timeline that @sgoldstein is currently handling and democratising that effort among engineering management of the Ops Section.
