Fix problems with CODEOWNERS and missing team entries

Why is this change being made?

  • To fix incorrect entries in .CODEOWNERS
    • Missing slash in directory path (without it files in the directory are not included)
    • Outdated paths
    • Usage of Slack name instead of GitLab name
  • To add missing GitLab names to team.yml
