Add job title and team to offboarding slack message

Why is this change being made?

The role and team of a person who is going through off-boarding was added in the merge request !40574 (merged), but then it was reverted in the merge request !36823 (merged). I suppose it happened because the second MR was created earlier but it was merged later than the first one, and the conflict was resolved incorrectly.

Could you please add it back?
