New Milestone process proposal

After noodling what has and hasn't worked, I am proposing this adjusted system for milestone planning. The primary difference here is that issues are collectively pointed independent of their assignee. This moves the Milestone planning meeting to the week before the milestone starts, for collective pointing. There is not milestone kick off meeting anymore. Milestone reflections can happen async, in writing.
