Update teleport runbook based on user feedback

The Teleport runbook had some language that was aspirational. It explained how we want things to work in the future, but in this initial phase things are more manual. I've updated that section with the current process. We can change it back later when the Slack integration is set up.

There are also a few minor corrections and clarifications.
