feat(visualization): Fix broken Postgres links in Grafana dashboards

Small improvements to the Tooling Links for the Patroni service.

  1. Fix incorrect index
  2. Adds failure matching
  3. Switches primary dimension for split visualizations to json.sql_state_code which may be more immediately helpful. See gitlab-com/gl-infra/production#3203 (comment 468910149) as an example of this.
