update ES upgrade issue template

Adjust the Elasticsearch upgrade issue template:

  • minor upgrades will not pause indexing and do a rolling upgrade (to reduce customer impact)
  • update rollback plan for minor upgrades
  • add some more alerts that we found needed to be silenced for when indexing is paused
