Update approval process to make it easier to understand and execute on the approval process for breaking changes

• Move approvals tracked for multiple people on one line to one person per line
• Changed approval for "VP of Support" which we don't have currently to "one director of support representing the others" as seeking approval from four different people is onerous. Example: Breaking change exception: Remove support for L... (#11524 - closed)
