Escaping ``` in preformatted text
It seems to me that it is currently impossible to write preformatted text in which a line starts with triple backquotes, making it for instance impossible to put examples of gemtext using them inside a preformatted text block. I'd suggest a slight modification to the preformatting toggle, which would avoid such problems: the preformatting toggle can be any number (at least 3) of backquotes (followed by an optional alt-text). However, switching off preformatted mode can only happen if the number of backquotes of the closing toggle is equal to the number of backquotes of the opening toggle. For instance, this allows to include triples backquotes inside a preformatted block in the following way:

I know this would mean that some extra code would be needed in clients to correctly support it. However, it seems to me that the extra code is quite limited, and the benefits are greater than the cost of this change (especially considering the specification is not frozen yet).
