feat!: support journal page anchors

M.A. requested to merge mkahvi/foundryvtt-pathfinder1:support-journal-page-anchors into master

Refactors the code to support page anchors.

Honestly with how much code this requires to support it and confusing it all, this might be undesirable change.

Some API was broken to accommodate adding anchor in the data and to clarify the purpose of the data, removing excess data where it was not helpful and few other tweaks.
