Remove field name entries

This makes the field entry names work better in translation. For example, instead of this:

### 7.1 Categories<a name="Categories"></a>

Field Name: `Categories`

The English source strings would look like:

### 7.1 _Categories_<a name="Categories"></a>

And the translations would look like:

### 7.1 分類 (_Categories_)<a name="Categories"></a>

That would render like:

7.1 _Categories_

7.1 分類 (Categories)
