Fix stuff in README.md

Addresses #30 (closed) and generally fixes some stuff with the README.md

One of these things is a basic table of contents so that there's some degree of organization, it could probably be better however that can be addressed at a later date. There's a lot of other small changes so I recommend reading the diffs to see if you like them.
