plan skill and the issue-first rule

Two things ported from planning the first milestone by hand.

  • skills/plan: the mirror of milestone. Reads what the group has, proposes milestone and issues in prose, and creates them with plan.sh from a JSON plan. Fixes the issue body shape and the planning rules.
  • CLAUDE.md: every piece of work starts from an issue under a group milestone, and how an ADR relates to milestones.

Nothing for the operator to run after the merge.
