Updates to HTML converter for CDLJ & CDLB

  • New footnotes are in an unstyled <ul>
  • Table cells have borders as indicated by LaTeX
  • Table cells spanning multiple columns are imported correctly
  • All (new & existing) headings (<h1>–<h6>) are displayed inline with the following paragraph (or as a block otherwise)
  • New reference lists are in an unstyled <ol> (see #1087 (closed))
