resolves #90 implement the default site generator component

  • add implementation and tests for default pipeline component
  • drop ./ prefix from image and attachment dirs set by AsciiDoc loader
  • rewrite tests in pipeline-default to use new repository builder
  • rename pipeline-default component to site-generator-default
  • rename page-generator component to page-composer
