Break edit-this-page out into separate partial

FeRD (Frank Dana) requested to merge ferdnyc/antora-ui-default:break-out-edit into master

I think the "edit this page" link is more likely than other parts of the toolbar to be a target of downstream customization. To facilitate that, break the page-editing section of toolbar.hbs out into a separate partial, edit-this-page.hbs, which toolbar.hbs includes.
