[#18] Add VistA DDL functions SQL definition to repo

Sam Habiel requested to merge shabiel/YDBOctoVistA:ddl18-vista-functions-sql into master

Whereas we have _YDBOCTOVISTAF.m, the actual functions are really not usable until we have their definitions. This commit supplies that file.

Manually tested on VistA using the queries located at YDBOcto#755 (closed). Eventually this file and the queries will be added to the pipeline, but this needs to be merged first.
